Providing Housing and Support Services

The Assertive Community Treatment (ACT) Team delivers comprehensive services to individuals who are diagnosed with mental illness and whose needs have not been well met by more traditional service delivery approaches.  ACT provides treatment, rehabilitation, case management and support services via a mobile, multi-disciplinary mental health treatment team using a highly individualized approach.
